Healthpacks spawned from bull will despawn early if they fall for a certain distance.
Easily reproducible in practice mode.

I don't think this is intentional.

It might be the same for Yuri's mines, but I can't really test it.

I haven't really messed with this engine in a while and a bool I remember being tied to this seems to be gone. However, I'd guess this is how the engine creates a 'distanceless' bullet now:

bullet:JetterMine_distance = -1
Bullet:CowboyBullHealthpack_distance = 1

They just need to set its distance value to -1 or to something astronomically high. Either fix is fine in practice.
